What do I need to do before my appointment?

Complete all patient forms located in the patient portals

Have a form of identification and medical insurance card available for the visit

Check to see what services are covered by your health insurance plan or if you need a referral from your primary-care provider prior to the day of service

Prepare a list of all medications and supplements you are taking to disclose the day of your appointment

If you are having a procedure preformed, ask about restrictions or cautions prior to the day of the procedure

There will be a $50.00 No Show/Same Day cancelation fee assessed if a patient misses the appointment and/or does not cancel the appointment at least 24 hours prior to the appointment time. That fee must be paid prior to scheduling another appointment with Carter Snell Skin Center. Insurance does not cover these fees.

Arrive on time for the appointment in order to avoid being rescheduled
What health insurance plans does CSSC accept?

We are a cash free office. All major debit/credit cards, including CareCredit ®, are accepted.
